Report an Injury

INJURY INFORMATION

Please complete the Ringette Ontario online injury report form if you, your child, or a member of the team you coach has been injured during a ringette-related activity.

We encourage members to report all injuries sustained during ringette-related activities.

Please note that Ringette Ontario has extended medical insurance available for individuals who sustain injuries. If you wish to submit an insurance claim for your injury, you must have completed a Sport Injury Form.  For all insurance claims, please use the Athlete Claim Report and submit it as per the instructions on the form.

Please note that all injuries must be reported within 10 days of the initial incident. Complete the injury report form here.
